$69/consult
45-60 minute consult on products and services offered, height, weight, and BMI with a customized medically supervised plan. To start injections or medications you must have fasting labs within the last 60 days, you may bring your labs to your visit or we will perform for a reduced fee of $95, you must fast for 8-10 hours if labs will be drawn at the visit. If you are not fasting the day of your visit, we will schedule an appointment for weight management fasting labs. Labs included: Fasting Lipid Profile, Fasting Insulin, CMP, CBCD, TSH, Vitamin D, Vitamin B12.
Gold Weight Loss Membership
$159/month
The GOLD Weight Loss membership includes a weekly weigh in visit with the nurse or medical assistant and a supplement/lipotropic injection with added vitamins each week. In addition, you will meet with a board-certified nurse practitioner every 4-8 weeks (telehealth provider visits are available for those living out of town). The InBody scan is included with your GOLD membership every 12 weeks. Plus, 10% off vitamins and supplements in office. Clients with a history of kidney stones are not a candidate for lipotropic injections. Semaglutide and Tirzepatide are available for those who qualify at an additional charge.

SELECT Weight Loss Select Membership
$79/month
The SELECT Weight Loss membership includes a weekly weigh in visit with the nurse or medical assistant (supplement/lipotropic injections are not included with this membership type, but you may purchase an injection at your visit). In addition, you will meet with a board-certified nurse practitioner every 4-8 weeks (telehealth provider visits are available for those living out of town). The InBody scan is included with your SELECT membership every 12 weeks. Plus, 10% off vitamins and supplements in office. Clients with a history of kidney stones are not a candidate for lipotropic injections.

BASIC Weight Loss Membership
$59/month
Weigh in once each week (not mandatory, but highly recommended) as a nurse visit in office or through
telehealth. Nurse practitioner visit 1x a month as requested, must have a provider visit every 8-12 weeks in person or through telehealth. Minimum of every 12 weeks face to face in office. InBody scan every 6 months. $10 for extra for each additional. InBody scans were started in 2024, if you have not had yours, please let us know. 10% off on vitamins/supplements and OBAGI skincare. Prescription medication is not included in the monthly weight loss membership; medication is in addition to your monthly membership.
NOT an option for controlled substance med refills or those getting weekly in office injections. Controlled substances do not qualify for telehealth, this must be a face to face visit every month.
Maintenance Weight Loss Membership
$39/month
The Maintenance Weight Loss membership includes a once-a-month weigh in visit with the nurse or medical assistant (supplement/lipotropic injections are not included with this membership type, but you may purchase an injection at your visit). In addition, you will meet with a board-certified nurse practitioner every 12 weeks to discuss how the maintenance phase of your weight loss is progressing (telehealth provider visits are available for those living out of town). Maintenance is only an option for those with a paid weight loss membership for a minimum of 6 months, who have maintained their weight loss for a minimum of 3 months. This is a great option for those who have lost weight and would like the accountability and continued support from Select Health. The InBody scan is included with your Maintenance membership every 12 weeks. Plus, 10% off vitamins and supplements in office. Clients with a history of kidney stones are not a candidate for lipotropic injections. Semaglutide and Tirzepatide are available for those who qualify at an additional charge.

MEMBERSHIP TERMS: All memberships carry a 1-month minimum agreement. Beyond the first month, there is not a contract or cancellation fee, it is month-to-month membership. After the first full month, a patient can change the membership plan if needed/desired, with a 7-day notice before the membership renews.
